Regional collaborative care model for stoma patients

This is the graduation project (service design project 2018.3-2018.7) of my master program, a regional collaborative care model designed for stoma patients in China.

This is also a medical project of the Hunan Provincial Health Commission. I cooperated with a medical company in Hunan Province as the only service designer of the project, leading the preliminary investigation, problem analysis, demand analysis, planning and strategy formulation stages.

Based on the feasibility, the information direction was selected, and a basic low-fidelity prototype and test plan were produced for medical company to go to the hospital for testing.

This project received full marks in the defense.
